Homeless Prevention Outreach Program

 

 

Our Homeless Prevention Outreach Program provides services for individuals or families in emergency of crisis. To be eligible for the assistance program certain factors are considered, including household income, employment status and the likelihood of achieving future independence. Applicants must also exhaust other available resources. Persons completing the application must be head of household or an adult responsible for payment of housing and utility bills.

 

We must have ALL of the required documents in order to process your application.  If you are not contacted after two weeks, you may contact the office at (843) 965-4037 to follow-up on your application. Calls are accepted between the hours of 10:00 am and 3:00 pm Monday through Wednesday.

Documentation needed for assistance:

1.      Valid Identification:  Driver’s License or State ID of the applicant and social security card of all household members over the age of 18.

2.      Dependent information:  Social Security card and Birth Certificate/ proof of guardianship.

3.      Income Verification:  Check Stubs (last 30 days), Food Stamps, Social Security, SSI, Child Support payments.

4.    Unemployment:  If you are not employed, please provide legal documentation stating you are unemployed. This can be obtained at your local South Carolina Employment and Security Commission Office.

5.    Utility Information:  Current and last 2 months’ payment history for utility bills.

6.    Rental Information:  Copy of lease and current rental statement from landlord.
